TEAMS

The Follow-up Coordination team will recruit, train, and mobilize a group of volunteers who will be responsible for extending the influence of Canby Cares beyond the day of the event.  As much as the Lord allows, this team is to build ongoing relationships with clients, so that the Canby Cares event inspires lives well into the future.
    

Objectives:

•    Administer an exit survey to asses the success of the fair and determine client needs
•    Develop a mentoring program that connects churches and church members with clients in need
•    Develop a volunteer base who will make sure the clients get to their follow-up visits
•    Follow up on patient appointments for on-going heath care.
